Below is what you need to complete your application. Please contact the LPN Career Center if you have any questions.

- Also available in the "Program Catalog" section for download or you may pick up a printed version.
- $75 application fee:
- Payable by: Check, Money Order, or Online found on our website, lpn.csiu.org
- High School Diploma/GED Certificate:
- Official high school transcripts must be sent directly to CSLPNCC by the issuing school.
- Visit www.diplomasender.com for GED certificates or any high schools which are no longer in existence.
- If you have college credits which you would like to be considered for transfer, follow each institution's policy to request an official transcript.
- Entrance Exam:
- We require every student to have taken the TEAS exam within the last two years.
- The TEAS exam is offered periodically in the Spring and Fall at the LPN Center, and can also be taken in an online, proctored format. We will follow up with information about available testing dates once your application is received.
- TEAS scores are sent directly to the LPN Center by ATI Testing. Please make sure you select our institution to receive your results.

- Letters of Recommendation:
- We require two letters of recommendation. Letters should be from anyone who is familiar with experiences you have had in nursing or healthcare. If you have had no experience in nursing or healthcare, please provide reference from individuals who can verify your character such as supervisors, teachers, or other professionals. Please do not provide references written by family members.
- Statement of Purpose:
- In a brief essay, describe your reasons for choosing a career in nursing and the personal experiences or qualities that have inspired your decision. Your response should be approximately one to two paragraphs (about 150–250 words) and written in complete sentences.
- (Opt.) Transfer Credits:
- You may submit documentation of prior completed education.
The Admissions, Academics, and Ethics Committee reviews all applications for Ta recommendation of admission. Please contact us for specific deadlines and admission decision dates.
